Thanks. I found I had to be more explicit. I had success when I added all of these.

"application/vnd.ms-word.document.macroEnabled.12",
"application/vnd.openxmlformats-officedocument.wordprocessingml.document",

"application/vnd.openxmlformats-officedocument.wordprocessingml.template",
