Reply to Rename image from Upload Form

Rename image from Upload Form  new!

As the title says, I'm trying to rename the image from a field on the upload form, but can't get it to work.

It works perfectly putting the name directly into upload.php but I need to be able to name each image.

Any pointers greatfully received.
